Guy cuts the throat of his captive.. The cut is surely deep enough to of hit the arteries? Yet there is a massive lack of blood! Even post mortem a cut to the throat will bleed out like hell!

There would still be enough blood pressure to cause arterial spurts even if he is dead after the initial stabbing as there is no way there is enough blood loss from those initial wounds to cause that much of a blood pressure drop.

I think those people who scream "fake!" just want it to be fake, somehow deep in their subconscious they cant take the fact that they are partaking of another man's degradation and death.

For me, I have no such problems. The world is an ugly place and people have died for much 'lesser' reasons.
