What is happening is he is reaching down to pull out what is called slips. Slips prevent the pipe from falling thur the hole in the floor during adding or screwing another pipe to the string of pipes being ran into the hole. The thing that crushes his head is called power tongs. Power tongs are basicly a huge pipe wrench that grabs one pipe and holds it while screwing another pipe to it. Pretty much the driller controlling everything off camera hit the wrong button.

its on a drilling rig floor. his head was crushed between the casing and a kelly bushing. no reason to put a head there (at all)! ...and why was there a camera running - not a usual function.
