The heart is located on the other side of your chest.
This is incorrect. It's basically centrally located in the chest, under the sternum and lungs, between the nips.

I wonder if the guy was in pain half way through, or if he just lost his emotional composure somewhat due to listening to the recital of the prayer and realising he really is fucked.
To me it looks like he's trying not to react to the physical pain rather than any mental anguish he may be feeling at the moment. Machismo is a big deal down there and you don't want to come out looking like a little bitch, especially when you know you're totally fucked.
