When the big one comes you'll feel an intense pain in your chest that may irradiate to your left arm, neck or mandible, these are the signals you are having a heart attack, the pain is ACUTE and incapacitating.

If you are about to have one in the next few hours or the following morning you may experience back pain that you would possibly not believe it is related to heart problems.
