i later read that he leaned too far forward while filming and lost his balance so therefore a tragic accident but he will always be remembered as the hoover dam suicide man.

No it's his shoe. he's wearing white socks, so you can see he loses one shoe whilst sliding down and the other flies off stage exit right on impact at the bottom, his head then does void itself as seen when the body settles after the rebound.
