I think he was just trying to say he was hungry in sign language. This would explain why he smeared his own blood on his face (was aiming for the mouth. Lack of brain made it hard)

Then, when it cuts for a moment and films the man tapping and rubbing his head, his head looks quite smooth and the brain matter jutting from the crown is no longer visible.

My assumption is that he began rubbing his head and it knocked the loose and exposed matter onto the pavement (some of which can be seen), but I'm just surprised that the wound itself and additional matter isn't visible in the second segment.

Correct, i merged the two parts together but started with the better quality one first showing the graphic head wound as a lot of people have a short attention span when it comes to watching vids online lol

He is having stroke-like symptoms from loss of blood to his frontal cortex, I would assume. He is likely not coherent, or lucid, thankfully for him. Following a brain trauma of this sort, patients are rarely aware of what is happening during the event. Actually not for periods prior and following the event, either.
