I would hope death came instantly, but imagine if the initial blow knocked you out then you became conscious for a brief moment while you bleed to death internally, but paralysed, blind and deaf and in the most intense pain you have ever known. You might think you were already dead and in hell.
I am not trying to make ridiculous speculation to be edgy, I think it is a possible reality one might survive such an accident for long enough to know about it.
