Maybe they forgot about the 1/7 acetylene rule or the pressure was above 15 psig but seeing some acetylene explosion vids you know the huge fireball explosion is hardly survivable at nearby range
I watched on YouTube yesterday the one that happened in China (TianJin), that was pretty cool to see.

They likely didn't fill the cylinder with the proper amount of acetone to dissolve the gas. Shits super unstable...
