I say REAL & she's very much thrashing about! Also, she couldn't have put herself in that position, what with her legs being tied, the details with whatever all is going on with her face (looks like a mask, like they wear in China & whatnot) & THEN, the handcuffs or zip ties?! NO WAY she did that to herself & it's every bit REAL, IMO. Horrible, HORRIBLE!!!! way to go...
