I've seen these pictures before! I believe the guy had a penis enlargement surgery and had a post-surgery infection that took a long, long time to go away.

I think the infected cut/line down the penis was made so that the skin could fit the hugely swollen penis inside of it without ripping/being too painful. Looks like that backfired!
