I can tell this guy was inexperienced.
See the rough hole in the middle of the disc? There should be a thin sheet metal plate (see pic) there for the lock nut to tighten onto. It's not there because he either used the lock nut intended for a grinding disc (this is a cutting disc), or didn't fit the required washer (pic below). This results in uneven distribution of clamping force on the thinner, lighter (and therefore faster) disc which then warps and explodes at high RPM.
also doesn't pay to buy cutting discs (or any other power tool bits, discs, blades etc) at the 99c store

I have had some of these 6" discs come apart and some of the full size 15" chop saw discs come apart.
Scary as hell,face shields and leather gloves have saved me so far.
Usually goes
1:stop tool.
2:make sure fingers are still attached.
3:change boxers.
