This can and does happen with brand new high quality disc's. It has happened to me many times, just not this bad. They also spin at 10,500 to 11,000 rpm so they can a do come apart often. Hope he didn't severe his facial nerve. Full face mask is needed when grinding/cutting with these tools.
must have forced the cutting pace. im told the spin limit are only correct if the cutting pace is not too hurried. but if you try pushing the spinning disc too much against what you are cutting, the disc is liable to bend, twist and break. Next time, let the disc bite at an easy pace. more time and electric consumption but its safer

But seeing that it was a handheld cutting disc, i betting more on the possibility that the user experienced an unsteady/loose grip while the disc was halfway through the thing he was cutting,

I didn't know what it was tried to think of how a CD could come flying out the CD player.

I can tell this guy was inexperienced.
See the rough hole in the middle of the disc? There should be a thin sheet metal plate (see pic) there for the lock nut to tighten onto. It's not there because he either used the lock nut intended for a grinding disc (this is a cutting disc), or didn't fit the required washer (pic below). This results in uneven distribution of clamping force on the thinner, lighter (and therefore faster) disc which then warps and explodes at high RPM.
