Not being able to afford healthcare isn't much of a excuse, doctors anywhere in the world would do something to help a gangrenous ball sack. No need to let it go for so long befor treatment ?
Gotta have some pretty poor hygiene to let something that that even start in the first place ??

If you go to an ER, they have to treat you whether or not you have insurance.
You get nasty letters and sometimes an anonymous source will pay your bill, or there is a charity department you can try.
