A 23-year-old male student presented to us with a one-month history of a right upper thigh swelling which grew gradually following a penetrating bullet injury to the upper thigh. After the injury, he did not receive any surgical intervention; instead, the wound was packed in the accident and emergency department. The swelling grew over one month to reach a size of a tennis ball (measuring roughly around 12x10 cm). It was associated with pain on the right foot.

Chop wounds to face - what a mess!

This is another case of domestic helper battery which ended fatally.There were multiple burns by a heated mettal at different stages of healing, Looped abraded contusions caused by an electric wire. Contusions and lacerations caused by fist boxing. The final fatal assault was done by shavel blows to the head causing multiple lacerations of the scalp.The victims attempts to defend herself resulted in defence lecerations. There were no fructure skull or intracranial haemorrhages. The lacerated wounds of the scalp and hands together with burned areas showed sever pyogenic infection. Death was attributed to septicaemia

Suicidal cut throat. The superficial cuts are hesitation marks.



Suicidal cut wrist. Again the superficial cuts are hesitation marks.





1- Cut wounds are characterized of being linear in shape and longer than deeper.The length of the wound has no relation to the length of the causative agent. The wound is usually deeper at its origin and tails off at the end.
2- Edges are even and cleanly cut except in case the skin is corrugated eg. scrotum,neck and axilla. Or in cases were the causative agent has uneven or serrated sharp edges eg. broken glass or serrated knifes.
3- Edges are everted and gapping of edges occur when the incision line cross Langer's lines or the underlying muscles are cut transversely.
4- Edges show no abrasions or bruises except in cases where the wound is inflicted by a heavy bladed object eg. meat_cleaver,axe or hatchet.
5- Angels are acute.
6- Hairs are sharply cut.
7- The base is regular and deeper tissues are cut in the same vertical plane with no tissue bridges.
8- Free external bleeding.
9- Less labile to sepsis due to less tissue destruction.absence of foreign material at the depth of the wound and prompt bleeding which washes any bacteria carried to the wound by the weapon.
10- Heals by primary intention with minimal scar formation.
Circumstances
a- Accidental :- Accidental cut wounds are common. It result from the use of knifes,broken glass and cutting machines. In traffic accidents,pieces of broken glass cause characteristic cut wounds having the shape of right angels. The site of such injuries can help in determining the position of the occupants in the vehicle.
b- Homicidal:- Incised wounds implies the intent because the instrument used is not commonly carried by ordinary person. Frequently the motive is to disfigure rather than to kill.
Fatal homicidal incised wounds are usually directed to the throat. Homicidal cut throat has the following criteria.
- Lacks tentative ( hesitation ) marks.
- Defense are seen on the palms of the victim or on the ulnar aspect of the forearm who is trying to grasp the weapon from the offender or trying to ward off the blows.
- Usually accompanied by deeper cuts else where in the body.
- Commonly lies in the lower neck (the victim usually tries to protect his neck by flexing it bringing his chin against upper chest). Homicidal cuts in the upper neck usually indicate pulling the head backwards by grasping the hair by the assailant and the attack took place from behind.
- The wound is more horizontal in direction.
- The wound has the same depth all through. The neck may be completely served.
- The cutting agent is usually absent from the scene.
c- Suicidal:- Suicidal cut wounds are more common among males than females. It usually involve areas where vital arteries or veins are superficial, eg. neck,wrist or the groin,
Suicidal cut throat has the following criteria.
- Always start with superficial cuts parallel to each other ( hesitation or tentative marks)
- In case of a right handed determined suicide, the wound begins high up on the left side of the neck to end on the right side of the neck taking an oblique direction.
- The wound is deep at its start point and tails off at its end.
- The act usually take place in solitude( in doors) and may be in front of a mirror.
- The victim usually take off his clothing to have a better access to the chosen area.
- The instrument used is found in the scene either near by the victim or grasped by his hand (cadaveric spasm).
- Shelving (different layers of tissues are not cut at the same level) is common since the blade is usually held at an angle to the surface.
d- Self inflicted cuts:- A part from suicidal cuts which are self inflicted,tow categories worth mentioning.
- Fabricated wounds. These are self inflicted superficial cuts made to falsely accuse somebody. It is not disfiguring or involve vital areas arranged in groups of parallel or inter-crossing cuts. It is made in sites within the reach of the right hand. Cloths are usually free from corresponding cuts.
- Self-mutilation:- These are serious deep cuts usually disfiguring or involve the external genitalia. Such injuries indicate sever psychotic derangement.
