With proper nutrition, minimal oral hygiene, and a fully functional immune system, it doesn't happen at all. Parts of Africa are severely affected because of immune suppression, some from malnutrition and some from HIV. It is treatable very early on, when the lesions are still sores inside the mouth, but once they have gotten big enough to start eating holes in your face, it would be difficult to treat without removing all the affected tissue.
