Not fake.
I would say it's the result of a infection that went septic and rotted the skin to the skull. The area around the skull is thick due to inflamation. The bloke himself...he looks completely nuts.
If that is 'fake', its a seriously good makeup job with clothes and the flies and the way the bloke is acting. Its the best fake iv ever seen.
