if pull knife out then he at risk of bleeding to death has while in situ it acting to some degree as a seal...also need to fully assess the damage prior to anything further taking place..hence the need to enter the chest wall through the method shown..repairs then carried out on a as required basis.

you cant just cut around or pull the knife out. you need to look at the damage done first and make sure when taking it out its not going to kill the person.

You can't just yank it out - risks the patient bleeding out and then you gotta stitch everything back up without screwing anything up further....he's lucky that he just didn't bleed out with that thing still in him
