What the heck is wrong with him?
Besides that horrid pressure ulcer I think the guy has been burned. Provided they're sanitized first, banana leaves work as well as Vaseline and gauze as a burn dressing. They're certainly more abundant down there than gauze.

Somebody needs to get the man a lifetime pass to the nearest opium den.
