I saw that. Fragments from it lodged in his neck and he stuck his thumb in it to stop the bleeding

My opinion.. something was lodged in that barrel. Or they loaded the wrong ammunition. That round didn't make it out of the chamber

This is what happens when you live in a hot and humid and don't clean and oil your guns regularly. Almost looks like they found this one at the bottom of a pond judging from the look of that stock, almost looks waterlogged. And he never stuck the barrel in the dirt that I could see, and would think it more likely he had a squib load than a little dirt plugging the barrel.
