Most of the more serious firearms failures such as the one purported in this thread are due to barrel obstructions.

One round is fired and, due to low gas pressure, becomes stuck in the barrel.

The next round fired now has to overcome the friction of two rounds - but - cannot do so before the gas pressure builds up to catastrophic levels and the firearm blows apart.

Firearms are engineered pretty precisely.

There is enough pressure to potentially blow up pretty much any firearm in every single round.

The trick is to make sure that the bullet leaves the barrel...just... before the gas pressure builds up too high.

Every single round is a 'balancing act'.

Best of intentions posting this but the pics are unrelated.

The damage to the thumb does not correlate to the guns fragmentation, I had a training class today and this picture actually came up as we discussed factory squib loads.
