the pics are definately backwards i have a friend who is having the opp done (M to F) and they've explained the whole procedure. that clear tube on pics 3 & 4 is what they use to make the viginal cavity by inverting the penis.

Also the reason why "Heshe" looks like a kinda maculine woman is because the patient would be made to live their life as the opposite sex for approx a year before having the actual opp. A woman would not be having a sex change to become a male...still looking like a female!
