I think it's real. Found this pic (wax model, probably), gives a better look though.

Yah I was going to say that there are 2 primary problems with this guy's legend: 1) conjoined twins can't form like that and be at all viable, and 2) there's no way the twin could be talking to him because it wouldn't have the ability to produce the breath necessary to speak. But it's an amusing myth.

That's the same model as shown in the original picture. Only difference is the inclusion of a new, more realistic(?) wig.
