I think he mentioned drinking something called 'Mountain Explosion and the caffeine from it thins the blood(which for what he was about to do is very bad). He seemed to want to protect his penis so I don't think it's a gender identity disorder so maybe it is some sort of cancer.

I watched it with the assumption that it is cancerous and he couldn't afford medical treatment, rather than is totally nuts, although that might still be the case. He is either very brave or very stupid. Perhaps both. I did watch it with the sound off though, so maybe I missed something.
