I had a Chihuahua come through a clinic I worked at with this. I literally grabbed hold of the plaque, pulled, and removed 5 teeth all at once, embedded in a bedrock of plaque.

I've seen that before! Crazy. I read some where that the patient is mentally handicapped and his/her caregiver was not taking care of them
