The foal had: One head, one set of shoulders, two sets of hind legs, one set of front legs, and two abdomens.

It came out alive, but died shortly after. The mare was fine.

The hooves look strange because they are initially soft and covered in a membrane so as to not harm the mare during pregnancy.

Sounds like just in humans, it was supposed to be two foals but the egg didn't fully split.
