I broke a blokes face in a fight with my elbow. One side of his face blew up, but not this bad. His sinuses were all fractured and the air was escaping to under the skin within minutes

Well if this is in fact real, all of the facial tissue would have had to have been disconnected from the muscle within. And although looking quite fake.. quite possibly could be real, considering some of the shit we see on here lol. Going to research this right now.

I've been trying to find a follow up on this guy for months.. We're using this video in some airway management emergency classes.

Motorcycle accident in Manila, Philippines.
Head and neck trauma. Trachea was fractured and air couldn't evacuate his mouth or nose. He's breathing, but exhaling is nearly impossible.The air rerouted itself through the soft palate of the mouth and into the eyes. He survived as he was able to keep himself calm enough to breathe until help arrived.
I know that's what LiveLeak said, it's not accurate. This is subcutaneous emphysema caused by a class III Le Forte panfacial fracture. Sometimes it's called an orbital blowout, his sinuses and nasal passages are crushed.

Man, I wish this video was longer, the severity here such a phenomenal event, I've shown this to my preceptors, and ICU staff, no one has ever seen a Le Fort of this proportion in real life. I've seen this on a way smaller scale, like a cheekbone after a fight and one eye socket on an unrestrained driver.

Questions:
1) Has the separation between his esophagus and trachea been breached? That's the only way I can think of that someone could breathe into their head area without a functioning trachea.
No, I believe his trachea is just fine. If there was a deviation below the epiglottis (where the trachea and esophagus separate), it's possible that exhaled air could escape into his esophagus, but it would fill up his stomach, then come right back out as a burp or vomit. That happens if an ET tube is placed wrong during intubation and not caught. This is sinus damage.

His throat looks untouched, there's no deviation or abrasions that would indicate trauma. And he's inhaling just fine, there's no accessory muscle use, he just doesn't present with tracheal/lower airway damage. His Adam's apple is pronounced and not deformed Remember, the trachea begins at the level of the collarbone, it's part of the lower airway. That's why I don't agree with tracheal damage.

He's getting adequate oxygen, he's got good color in his earlobes and lips. It's almost like he's demonstrating for the camera what happens when he tries to exhale through his nose.

It really is a beautiful injury. If his spine and brain escaped damage, Balloon-Boy here is going to be just fine after a surgery. As long as he doesn't go into a traumatic cardiac arrest. He probably won't be as pretty, but it's not THAT THAT THAT bad. How come I don't get any calls this awesome. Lately I get is old people who fall down.

Im no doctor but i seen this happen at work not that fucked up but . He got hit in the face and fractured between his eye and around his nose so if he breathed heavy it would do the same but as i said wasnt next level like this

Facial fracture at the sinuses.
Kind of like a pneumothorax of the face.
Wish I had more calls like this instead of saving grandma from the gravity monster.

I bet that its special effects makeup. On side away from camera he has a pump of some sort. An injury of that level to cause instantaneous swelling would not go back down like that. I saw no huge explosion of liquid/blood pus. No way its real. But its good . very good. Makes your stomach curl until intelligent reasoning takes over!!! But its damn good!

Ok so reading above..i guess i am most likely wrong. I read most of the comments but dudnt see the few that had medical expertise sounding info. So i defer to the smart guys. Thats so gross. I would die if that happened to me ....but it is a crazy unbelievable injury. Thanks Medical Smarties fir teaching me a thing or two! I will read comments more thoroughly before spouting out my dumbness for all to see!!!!
