I think it's real. For one, just suspending most of your weight will constrict breathing enough to pass out, death follows soon after. In this case, though, she didn't compress the larynx, but the rope is very tight, enough to compress the carotid arteries and jugular veins. So she could breathe for a short while, but it's a moot point if brain is not getting blood. I vote real. Another point, no movement at all of body between different camera angles.
