I say he climbed up onto the wheel chair to tie the rope to roof. Was not handicapped at all we just surmise that because one is there.
That and he is downstairs. I don't see any indication of a lift or elevator.

Weirder, is he wound up in a semi kneeling position facing a picture on the wall that appears to have metals below as if it is a shrine of someone he cared about???

If you look closely at the table it appears the chair went thru the table then was repositioned to a more secure part of the table . I would guess he could at least stand briefly to tie the top knot. Notice the foot rest is up. A disabled person could sit on the floor and push the chair onto the table then crawl/ pull themselves up into chair . My guess is he had plenty of time and had some strength in his legs, and had good arm strength. With those abilities you could loop the sheet over the pipe then pull your self to a standing position to tie the knot around pipe and around neck then sit. Doable but determined.
