I'm thinking the mark on the forehead was from the butt of the knife that was used. Maybe the kids were killed first and of corse the mom put up quite the fight.

This was a message to others who might have owed the loan sharks money. Think about how quickly those late payments were paid by those who were behind on their payments, a very good motivating tool.
