I have seen this photo cropped a little differently. If you look closely at the right "eye-ball" you can see that what has happened, is that both of the eye-balls have popped out of the socket, and are being suspended by the optic nerves on both sides. You can see the little round thing at the end of the nerve on the right side, but the left side has been cropped out of this particular photo, but it is visible on other photos of this person.
