That's seems to be part of a child's head, the 6th passenger other people are mentioning. I think her whole head is in the back.

I was trying to work out just how many people were in that car. I saw check shirt man and green top lady in the front, then in the back is orange top, pink top and Ms Stretchy Neck.. Then there was an extra face in Ms Pink's lap and what looked like a head in the boot.. I thought the lap head was a child to begin with, and Im pretty sure it is because in pics 1,2 and 4 there's a very small leg resting on check shirt's shoulders...
