Abdominal ascites resulting from liver cirrhosis. The affected liver hardens and doesn’t allow enough blood to pass through resulting in portal hypertension, along with a decrease in the liver’s production of albumin which is a ‘magnet’ which withholds water in the venous system, and reduction in kidney function to remove excess water — all causes buildup of fluids in the peritoneum (stomach), producing that pregnant look.

Alcohol poisoning is usually described in the accute-sense like drinking a whole body of whisky by yourself. Her liver gave out prior, the secondary long-term effects of portal hypertension, ascites, and esophageal varices killed her.

Soiled-mattress was most likely from the immediate cause of death which was esophageal varices (veinous rupture in the portal system) resulting from liver failure. Blood comes out of the rear-end as tarry/bloody (uncontrollable like rear-end vomit) stools, and likely bloody vomit. She may have been like that in bed for a few days. Many patients in this situation are nude as a reaction to self-sanitary reasons mentioned above. They know what’s going on, they’ve been advised by their doctor what’s likely to happen, are likely on the liver transplant list, but have also accepted the likelihood that this is how they are about to die

The rectum relaxes open after death, and is commonly confused with sexual assault by those unfamiliar with the death process. The blood was rectal bleed out from blown portal vein resulting from hypertension resulting from liver failure. The hemorrhoids are common with liver failure patients and are the result of portal hypertension

Esophageal varices, resulting from portal hypertension, resulting from liver failure. She’s acutely bleeding out from both ends of her body in the form of bloody stools and bloody vomit
