Is there a noose at all? For me is looks to be only a U shaped scarf, on which he puts her neck, holding on with her fingers. I don`t see why she would not just fall backwards to the ground. But on Chinese movies this is often this way hanging takes place, without a real tied noose...or am I completely wrong?

The scarf essentially gets “hooked” behind the jaw joint. The hands are either trapped as she lowered herself or went there after she blacked out. Either way they didn’t gave the strength to do any good.
