The smell isn't horrible but you smell the sickness.

What you can smell is the piss and feces (possibly in a diaper) the urine will have a special odour if you have pancreatic or liver cancer.

1. That was his wife or girlfriend.
2 without proper palliatic care, tumors, especially on throat, or mouth ones are phisically makes you incapable of eating anything. Next is the constant vomiting, this particular dude had a huge tumor on his throat, but you cannot see most of them...if you have not see a big ass tumor it doesn't mean it cannot exist...
3. Almost every terminally ill cancer patient will be starved to death, because this shit eat away your healthy cells, most of the food feeds the cancerous cells, not the other ones.
4 I'd say this dude also had a secunder liver tumour, look at how yellow he became...not to mention the osteoporosis caused by his primer tumor (got metastatic quickly i guess)

few months, sometimes weeks. Enomous weight loss is the first sign of "not good" which (without proper treatment) will quickly turn into " you are a goner" status. It's called ECOG, ecog 0 is where you can dance and jive, 1 where you are getting weaker, 2 when you need constant help to go on with your life, 3 is where you are pretty fucked and you are mostly in bed or a hospital, 4 is this guy. And yes, liquid food (special, turbo ones with full of nutrition and energy plus vitamins) are the basic neccesity, if you want to live beyond a few weeks
