For those of you who don't know, that's a stoma bag. He's had an ascending colonoscopy, likely from colorectal cancer, whereby they've removed the transverse, descending and sigmoid colon sections. In such a surgery, it's not uncommon for the anus to be surgically closed as it is no longer used and prevents infection. By the condition of his health, he probably gave up treatment.

I have a friend who is pretty much in this condition now. No meds are working anymore. They've removed most of her intenstine. Her scar popped open the other week and what was left of intestine popped out. NHS

My grandfather had colon cancer when I was very young. He wears a bag on his stomach but I've never seen it, so I don't know whether it looks like the one pictured. But I'd say, like others in this thread, it was colon cancer.
