Ostomy bags can be a bitch to put on & maintain leak-free. I once cared for a patient who had two, a regular colostomy plus another over hole the cancer had eaten right through his belly. Poor bastard.

It definitely shouldn't. It looks like neglect, though the fact that this is in a Cambodian boarding house (?) may suggest he was an addict and/or self-destructive from the outset.

Probably some type of CA. Looks like he has port for IV meds in the left upper chest towards his mid collar bone. Sad....
