reminds me to have my pistol checked under microscope for cracks. been more than a year since the last check where they recommended to have the barrel replaced, ballistic checked and re-registered.

The bullet may have stuck in the barrel when she ejected the dud round then loaded a live round in to the chamber with that bullet in front--pow--lucky to be alive. |

That was a WWII or Korean War vintage M-1 Garand...they're made like a tank. I feel pretty certain that it was a dud round that lodged in the barrel. This was an ammo issue, not a firearm issue. It was either a dud or a misloaded/hot round. That particular gun is renowned for not doing what it just did without a catastrophic ammunition failure.
