Ok, this is an ectopic pregnancy, and that is why both the mom and baby died. Notice the uterus to the left, and the baby attached to the fallopian tube. This could have been prevented had the mom had an early ultrasound. The baby is 14-15 weeks gestation. The mother would have died from blood loss from the rupture of the fallopian tube
