Nah, the 'over-exposed' area looks to me to be more of a pseudo censoring of the actual cutting area. Notice that it also appears on top of the arms and hands of the doctor, if it had been due to a bright light, it would caused shadows and most likely wouldn't have been aimed on his hands and arms. Somewhere along the way this has been censored for some reason....
