That's a M203 Grenade Launcher which was very common in Nam, usually in conjunction with the M16.
In all likelihood the Ukrainian Soldiers got them as part of an aid package from the US as there would be no shortage of them that's for sure.

I've never seen an M203 mounted on an 'AK', or muzzle loaded for that matter.
Are you sure he's not using a Russian GP series launcher?
