I hear in the case of this Russian incident it was from poorly maintained equipment, computer related. I'm not really an expert so can't answer your question definitively. Maybe if this was a US thread I would look into it for you.  From a defence reporter of the Kyiv Independent with a simple statement: "Breaking through the line"
