Proton-M Crashes Seconds After July 2013 Launch

A Proton-M/DM-03 carrying three GLONASS satellites failed shortly after liftoff in July 2013.

Within seconds, the booster began pitching left and right along its vertical axis. The onboard guidance computer attempted to correct the trajectory, but the vehicle entered an unrecoverable pitchover.

The upper stages and payload separated 24 seconds after liftoff under the forces involved. The first stage then broke apart and erupted in flames. The vehicle struck the ground 30 seconds after liftoff.
