NOAA-N Prime Satellite Topples During Lift, 2003
In 2003, the NOAA-N Prime satellite toppled while it was being hoisted into a vertical position on a platform, causing about $135 million in damage. A NASA investigation found that 24 bolts had been removed from the platform without documentation before the satellite was mounted. The missing bolts were not checked before the lift.
