Improperly Secured Insulation Stack Collapses in Warehouse
In 2019, a stack of insulation collapsed in a warehouse after being stacked too high and improperly secured. The warehouse normally held about $1 million in inventory, but was carrying about $3 million in returns and delayed outbound orders during inclement weather. A driver had taken insulation from the area when the stack began to fall. The collapse damaged overhead lights, which were not caged. The facility later upgraded the lighting to LEDs.
