Goiânia Radiotherapy Source Contamination, 1987
On September 13, 1987, a forgotten radiotherapy source was removed from an abandoned hospital site in Goiânia, Goiás, Brazil. The radioactive capsule was subsequently handled by many people, causing a major contamination incident. Four people died. About 112,000 people were examined for radioactive contamination, and 249 were found to have been contaminated.  ·
