I used to rebuild engines for a living, and this type of failure is actually pretty common across the whole spectrum of car engines. It's called "windowing the block" when it happens.
seen a guy shoot a lot of nitrous in a factory V8 toyota motor, and threw almost all the rods. it was the holy block till the core guy took it.

We had a Caterpillar 980 shovel that broke a seal on the turbo. When the driver tried to turn it off at the key, nothing happened.
It seems that the engine was no longer just running on diesel, but was scavenging oil from the 10 gallon oil sump.
The fucking thing got faster and faster, over running the engine governor and the smoke from the exhaust stack was up around 500 feet! I was all for letting the fucking thing go bang and stick a leg or two out of bed (same as windowing) and claim for a new engine, but a chap took off his Hi Vis coat and climbed onto the machine, took off the dust filter and shoved it down the hole.
That damn machine sucked it into the engine and slowly choked off the air and it stopped. Good fun to watch though.

Not too many people brave or knowledgeable enough to risk their ass to smother a runaway.
