Starting 1:45 the flames change from calm white to not-so-calm yellow. My guess is the floor coating catches fire then. Perhaps a coal tar epoxy coating.

Why wouldn't he just go a foot lower to be sure it's unhooked?
Repetitive fatigue or distraction. The crane operator probably did it right 1000s of times prior.

It would have been simple to add limit switches to give the crane operator a red/green light or lock down the crane if was not clear.
