US-Bangla Airlines Flight 211 With Mentally Unstable Pilot Crashes in Nepal

US-Bangla Airlines Flight 211, a scheduled international passenger flight traveling from Dhaka, Bangladesh, to Kathmandu, Nepal, tragically crashed during landing on March 12, 2018, resulting in the loss of 51 lives out of the 71 people on board.

The aircraft involved was a 76-seat Bombardier Q400 operated by US-Bangla Airlines. After the crash, it burst into flames, and the 20 survivors among the passengers sustained serious injuries from both the impact and the ensuing fire.

A year after this devastating crash, new evidence has emerged, confirming that the captain was mentally unstable and unfit to fly. Unfortunately, this revelation brings little comfort to the families of the 51 individuals who perished in an incident that could have been prevented if safety standards had been followed.

The captain, Capt Abid Sultan, exhibited erratic behavior and mental instability even before the flight took off from Dhaka to Kathmandu on March 12, 2018. The Cockpit Voice Recorder (CVR) transcript revealed that his mental state was more troubling than initially indicated in an Air Traffic Control (ATC) tape.

During the flight, Capt Sultan engaged in angry monologues about another female pilot, Farhat Lamia Khaled, whom he had trained. His obsession with her alleged comments questioning his competence as an instructor was apparent. His hostility toward Lamia extended to personal attacks, with derogatory remarks about her character and appearance, often laced with offensive language.

Mid-flight, Capt Sultan broke down emotionally, openly weeping and informing his co-pilot that he intended to leave the company due to his obsession with Lamia and rumors about their involvement.

As the flight neared its destination, Capt Sultan temporarily regained composure, providing instructions to the trainee pilot for approach and go-around procedures for Kathmandu. However, his behavior continued to fluctuate, alternating between politeness and compliments to his co-pilot's performance and abusive tirades.

During the descent, Capt Sultan mistakenly believed the VOR beacon was ahead of the aircraft when, in reality, the plane was flying northeast of the airfield towards Shivapuri mountain. Miscommunication, language issues, and a lack of assertiveness contributed to the confusion between the cockpit and air traffic controllers.

The Enhanced Ground Proximity Warning System (EGPWS) sounded repeatedly as the aircraft approached Shivapuri. Capt Sultan initiated a steep turning climb to avoid collision with the mountain. The air traffic controllers failed to provide timely warnings or guide the aircraft back on course.

Ultimately, the plane narrowly avoided a crash with the tower and other aircraft on the runway. It flew over the international apron, narrowly missing several other planes before its right landing gear hit the tarmac and veered off the runway.

Most fatalities occurred on the left side of the aircraft, while survivors were primarily located on the right side or towards the front.

The Investigation Committee Report concluded that Capt Sultan's stress and emotional instability, combined with deviations from standard operating procedures, led to a loss of situational awareness, preventing the crew from recognizing the aircraft's deviation from its intended path.

In the final moments recorded by the CVR, numerous audio warnings sounded, including "Terrain, Terrain," "Sink Rate, Pull Up, Pull Up," and "Bank Angle, Bank Angle." The radio altimeter issued "100 Minimums" and "50" warnings, with FO Rashid sounding alarmed with the last words heard: "Sir, Sir!"

The capt. was a real nut job Alot of other failures in the system and whole approach.
