TransAsia Flight 235 Hits Bridge in Taiwan Killing 43 People
On February 4, 2015, TransAsia Airways Flight 235, a domestic flight from Taipei to Kinmen, Taiwan, experienced a tragic crash. The aircraft, an ATR 72-600, had 58 people onboard, including 53 passengers and five crew members. Shortly after departing from Taipei Songshan Airport, a malfunction in the autofeather unit of the number-2 engine led to its shutdown. Unfortunately, the flight crew misdiagnosed the issue and mistakenly shut down the functioning number-1 engine. This critical error caused the aircraft to lose power, descend, and eventually crash into the Keelung River, near Taipei. The aircraft first clipped a taxi on the Huandong Viaduct, causing minor injuries to two people in the taxi, before crashing into the river. This dramatic sequence of events was captured by dashcam footage from several vehicles on the viaduct. Of the 58 individuals on board, only 15 survived the crash. The final investigation report highlighted a series of missteps and failures in protocol by the flight crew. The pilots failed to perform the necessary failure identification procedures and did not adhere to standard operating procedures. This led to confusion regarding the nature of the engine problem and resulted in the shutting down of the operative engine. The report also noted issues with the aircraft's autofeather unit, attributed to compromised soldering joints. Additionally, the crew's ineffective coordination, communication, and management of the situation significantly compromised the safety of the flight, ultimately leading to the tragic crash. |
