On August 6, 2005, Tuninter Flight 1153, an international flight from Bari International Airport, Italy, to Djerba-Zarzis Airport, Tunisia, met with a tragic fate when it ditched into the Mediterranean Sea, about 18 miles from Palermo, Italy. The accident involving the ATR 72 aircraft, designated TS-LBB, resulted in the loss of 16 lives out of the 39 people on board. The primary cause of this disastrous event was identified as a critical fuel system error. The aircraft had been erroneously fitted with a Fuel Quantity Indicator (FQI) intended for the smaller ATR 42 model during a maintenance procedure. This mismatch led to inaccurate fuel readings, causing both engines to fail due to fuel exhaustion, forcing the aircraft to ditch at sea off the coast of Sicily. An investigation into the crash revealed that this error was compounded by several factors. The maintenance crew's installation of an incorrect fuel gauge for the ATR 72, compounded by the similarity in design between the gauges for the ATR 42 and ATR 72, was a critical error. Additionally, the flight crew committed significant operational mistakes, exacerbating the situation and contributing to the eventual crash. In response to this tragedy, the Italian government took immediate regulatory action. Tuninter was banned from operating within Italian airspace, reflecting the severity of the oversight and operational failures. Furthermore, in a landmark legal ruling in March 2009, an Italian court found the pilot guilty of manslaughter. The court sentenced him to 10 years in prison, citing his failure to follow basic emergency procedures that might have enabled the aircraft to reach Punta Raisi Airport near Palermo. |

It crashed because on the plane (an ATR 72) has been installed an ATR 42 gauge for the fuel (FQI) designed for a model with smaller fuel tanks: it indicated the wrong amount of fuel, so the crew thought to be able to reach Djerba, but they were very wrong. The Captain (same dude who was flying during the incident) the day before, during another flight noticed that the FQI was not working correctly and reported it: but nothing has been done to fix it because the data in the database, due to some issue with a code (string) indicated mistakenly that the model could be used on both ATR 42 and ATR 72. So the last (VERY last) fueling was based on incorrect readings from the FQI: in Bari, the FQI indicated 2,700 kilograms, but actually just 540 kilograms were in the tanks ![]() Of course, the Italian government banned Tuninter from flying into Italian airspace right after that, I guess there's no need to explain the why. ![]() But also the pilot was found guilty for failing to follow the basic emergency procedures which could have brought the plane to reach Punta raisi airport, near Palermo: he was sentenced to 10 years in jail in March 2009 by an Italian court for manslaughter.
